The Lexington Clinic is only responsible for obtaining pre-authorization of the procedure based on medical information. It is the patient responsibility to verify their benefits, whether or not a second opinion is necessary, if the procedure is scheduled at a participating facility or whether their condition is pre-existing. Prior to having their exam performed, the patient/guarantor must read and sign the Patient Information Section of the Pre-Authorization Form (PAF).
